Poking at /var/log/messages I have (perhaps) a few interesting bits (I guess):
May 8 18:36:55 server pulseaudio[13972]: main.c: pa_pid_file_create() failed.
May 8 18:40:21 server klogd: hub 2-2.1:1.0: cannot reset port 1 (err = -62)
May 8 18:40:21 server syslog-ng[8647]: last message repeated 4 times
May 8 18:40:21 server klogd: hub 2-2.1:1.0: Cannot enable port 1. Maybe the
USB cable is bad?
May 8 18:40:21 server klogd: hub 2-2.1:1.0: cannot disable port 1 (err = -62)
May 8 18:40:21 server klogd: usb 2-2: USB disconnect, address 6
May 8 18:40:21 server klogd: usb 2-2.1: USB disconnect, address 7

But - I assume this is me just following my daily routine of turning the power
off on the monitor as I knock off work (it is a USB hub). It seems to happen
fairly regularly in the log with few apparent ill effects.
Hmm - sudden hardware failure [~yesterday] and nothing interesting in the
sysylog.
