I solved it on my machine with: gnome-keyboard-properties
There was us.map.gz in the list, as opposed to any other moment when I checked. Normally the keyboard type is more verbose (like USA, or USA with Euro)
I added USA and then removed us.map.gz and the problem disappeared.
Remains the question what put the wrong setting there.
Yep, but it's going to be hard to know how it ended up there :/ My current theory is that gdm put this as default layout because of a bug, and it ended up in your config because of this.
