For tools at least 5 years old, ext4, XFS, and Btrfs all default to 4KiB block sizes.
OK so I went back to RHEL 2.1 circa 2003 with kernel 2.4.9 and e2fsprogs feb 2002. By default the installer uses ext3 with a 4KiB block size. 'mke2fs /dev/hdb' results in ext2 with 4KiB block size, so that's the default for ~13 years. However, I was allowed to format with blocksize 1KiB and I could read and write to it.
