(*) it is quite unpredictable which window will get the focus when you do things like switch the desktop with keystrokes, quit (dialog) windows etc. Most often it is *not* the window the mouse cursor actually is in, and then you first have to move it out and in again....
I'm using KDE 4.4.3 with "focus follows mouse" with a delay of .5 s, and I don't have any problems with it that I'm aware of. Could you post the bug report number?
