Hmmm, judging from the log it is also looks as if it it never got an IP address assigned etc. I wonder if PPPD_OPTIONS overrides all the pppd options ... AFAIK, the main pppd options are from /etc/ppp/options, but I had expected smpppd to consider PPPD_OPTIONS as an override, and combine them with the default.
