I believe that is how I have my desktop setup. Admittedly it is not KDE 4.3, but it "just worked" when I moved from KDE 3.5.9/10 to KDE 4.2. When I get to my desktop, I can send you whatever configuration you might find helpful. I am using the "nvidia" driver, but a rather old version of it.

I can work with you on it a bit, but I'm not an expert of Xinerama, Twinview, or KDE's support for them and how it works under the covers. I got pretty good resolving issues for a while there under KDE 3.5, but I haven't had anything about Twinview/Xinerama break since installing KDE 4.2.
P.S. separate X-screens setup doesn't work either, with or without xinerama.
I've gotten Xinerama to work. I don't think Twinview is any better for my usage; probably worse.
