Not terribly obvious, but it takes TWO [or more] filters:
Absolute top-most filter: criteria: "size" "greater than" "1" action: "pipe through command" "spamc" (if using spamd, otherwise see the spamassassin documentation for the flags you would need) stop if matched: "no" [uncheck the box]
note: recent posts to the kmail list suggest "size < 230000" as spamassassin chokes on large messages [hangs kmail in the process...]
Second rule criteria: "x-spam-flag" "contains" "YES" (alternate) criteria: "X-spam-level" "greater or equal" "*****" action: dispose of properly ;)
Spamassassin re-writes the message with additional headers which you can check in subsequent rules. "x-spam-flag" is set to YES if spamassassin "believes" the message to be spam based on the config file, "x-spam-level" contains one asterisk for the "level" detected, so you can allow marginal spam to arrive by using a spam-level greater than what spamassassin triggers on
