There are two parts need for the mouse. Your InputDevice section looks OK to me but you also need to reference the mouse in the ServerLayout section. Here is how Sax2 set up my mouse (minus the Wacom tablet and PS/2 mouse that I edited out, three mice is really too many!) If you run Sax2 and let it setup the mouse you will probably have few problems than trying to follow an out of date How-to.
