You have to enable the loopback-interface for INPUT, too.

$IPTABLES -A OUTPUT -p ALL -s $LO_IP -j ACCEPT
$IPTABLES -A OUTPUT -p ALL -s $LAN_IP -j ACCEPT
$IPTABLES -A OUTPUT -p ALL -s $INET_IP -j ACCEPT

For me works :

iptables -A INPUT -i lo -j ACCEPT
iptables -A OUTPUT -o lo -j ACCEPT
