"It does not happen here." would be acceptable or bloat? :) Anyway, there could be two icons (one smaller than the other) because KDE4 reads the panel config and finds an icon-plasmoid set for konq. Adding to that KDE4 (I think it's the task-manager) started with some version to automatically place starters for the default filemanager and browser on the panel. Hence this could explain the second one. To remove any of those right-click and remove. To add a starter, right-click an app while it is active and click on "show a launcher when not running".
