I have found that the Menu Editor can really make a mess of things. Check out ~/.config/menus/applications-kmenuedit.menu. It's an XML file and is fairly easy to read. Try fixing it manually.
If all else fails, delete it and your menu will go back to default. Then use Menu Editor and the file will come back.
