(assuming that my file naming conventions are acceptable):

$ cat /etc/modprobe.d/50-iwlwifi.conf
options iwlwifi bt_coex_active=0

On Intel chipset, please try adding:
options iwlwifi bt_coex_active=0

to /etc/modprobe.d/iwlwifi.conf

Consider the following log, I think bluetooth failed around 21:53:54, what
could be going on here?  Is this an usb error or bluetooth specific?  What
information do I need to track this down?

After a reboot, I bluetooth failed pretty quickly, I see:
