All that is rather complex....
The easiest way to avoid the problem is to comment the lines in
/usr/lib/udev/rules.d/64-md-raid.rules (though that file might be in
/lib/udev/rules.d, depending on version) which mentions "/sbin/mdadm
--incremental"

That should make it work reliably.

The problem seems to be that the "udevadm settle" call in boot.md doesn't
always wait until udev has settled, but I cannot find any possible reason why.

(Thanks for the logs Christian - unfortunately it doesn't show anything useful,
which suggests I was on the wrong track).
