An update - yesterday evening, I started seeing CPU hardware errors (machine
checks), so we had the system board replaced. The system still only uses md0
for booting from, but now at least I get an md1 and md2 at bootup. Probably
just a coincidence.

I uncommented the last two lines when I was trying to figure out what was going
on, but leaving them in does not seem to make a difference.
