Huhh, that must have happened by accident; two times! To me it is no good idea to intermangle failsafe with normal boot entries. You normally do not need the failsafe entry except perhaps directly after an unsuccessful primary installation. On the other hand you may need to switch quite often between other boot entries like desktop/xen/factory-kernel and so on. The failsafe entries should be collected together at the bottom.
