Yes, something is resetting the SCSI bus. It could be faulty hardware. Or If MD
RAID reads beyond disk, the driver could trigger a reset when it detects this

To me this looks like MD is wrongly mapping logical block 2249 on md0 beyond
the disk! Probably Neil could explain whether and why MD reads beyond disk.
