I've found a lead/workaround/solution for the problem:
After booting the kernel with a different vesafb mode (0x324 = 1280x1024
32bit?) instead of 0x345 (1400x1050?) the DVI-D_1 works as expected.
I've include the output of 'X -logverbose 7' while running 'xrandr --output
DVI-D_1' while booted from the two different vesafb modes as attachments to the
case.
When running the vesafb in 1400x1050 the text on the linux console was a little
bit off the screen to the left so a decided to try a different vesafb mode. As
a plesant bonus the DVI-D_1 monitor became active both in vesafb mode and when
starting X.
