I wanted to update one image, and whatever I did it would show old images. At the end I have 2 uploads and 1 reversal.

Konqueror here.
Please check if you have read permissions on the file you want to upload.
For whatever reason, Konqueror (at least in KDE3) does not warn about "permission denied", but uploads an empty file... (Testing this in current KDE4 and opening a bugreport if this behaviour is still reproducable might be a good idea ;-)
