Yes, you should have a /boot partition located entirely before the 1024th cylinder (unless / can fit entirely within this boundary).

This is a rather difficult question without knowing how big your disk is or what the system is going to do. For a simple box I would create 5 partitions such that Windows system goes on the first and /boot on the second. The remaining three partitions would be for /, swap, and a fat16 partition that can be shared by both operating systems. Optionally you can add partitions to share between the OSes or to keep separate. As far as installation goes, I've always had luck installing Win first Linux second and putting lilo on the MBR and adding an entry to boot /dev/hda1 for windows.
