I always start in /var/log/, particularly in /var/log/journal/ if it exists. I
   du -ch /var/log/

Check the /var/cache/zypp/packages/ tree.

How many kernels are installed? An installation that old ought to be fine with
only one, two at the most.

Is old stuff accumulating in /tmp/?

/tmp gets cleaned out routinely even root.
cd /tmp ; rm -rf *   then reboot.
