We have just published a survey on YaST, our systems management and installation framework.
If you use any of the distributions openSUSE, SUSE Linux, SUSE Linux Enterprise Desktop or SUSE Linux Enterprise Server, I encourage you to participate in our survey to support us improving YaST. The survey will be online until mid November and the results will be published on openSUSE.org.

I second James' idea. Having the _option_ to enter name and mail address would be quite useful if questions arise. I doubt people will give different answers if they can enter name and mail address afterwards (at least in technical surveys like this - results might differ if the Alcoholics Anonymous run a survey ;-) Or you use the old trick of address collectors: "Want to win $hardware? Enter your address here!" BTW: At some questions, I had to select the "least wrong" instead of the "right" answer. IMHO it would be useful to have an "additional comments" input area at each survey question for cases like this.
