I saw something recently lately but I blamed a crash of my akonadi database. No problem when sending a test email to myself with kmail 5.13.1 (19.12.1). Also no problem when explicitly selecting iso-8859-1 encoding ? which by the way is a rather old encoding, probably UTF-8 should be used but iso-8859-15 is at least including ? :)

An Ü is displayed as Ãœ., I can't find an Ö and an Ä right now. *If you answer* the emails, the letters are encoded correctly in the editor window of kmail! It does not help to change the encoding in the kmail options. I _guess_ that there is an encoding error on the outlook side, but until recently kmail was able to deal with it.
