Is the connection configured to autoconnect? What's the output of
`nmcli connection show $CONNECTION_NAME`?

Btw, NetworkManager related logs will be very helpful. You can collect them by
adding the following to /etc/NetworkManager/NetworkManager.conf

[logging]
level=trace

reproduce the issue and attach the output of `journalctl -b` or
`journalctl -b -u NetworkManager` which only contains logs from NetworkManager.
