Thanks for report. I found that problem is in getting devices for /dev/dm-*
devices. Can you please try to enable also debug logs? simple pass on kernel
command line when starting installation y2debug=1 and it will be added to logs.
It looks like something is not properly recognized.

I will try to simulate it in our testsuite with your disk layout data when I
get it from debug output.
