As far as I can infer from the information you provided, it seems that some kinds of storage failure is occurred. Storage failure includes storage controller failures, (HDD/SSD) drive failures, RAID array failures, or simply kernel bugs, etc. In any case, as the cause cannot be investigated from the VM inside, please contact your hardware vendor and UCS support to check possible reason.
