As I already mentioned in the submit request, the likely reason for no
old-style icons showing up is that xembedsniproxy crashed.

Can you check whether it is running in that case?
ps ux | grep xembedsniproxy

If not, please run it in a konsole.
Does it crash?
What's the output?

@Fabian: disabling the patches may make it hard to examine the problem.
It might be caused by certain specific applications (wouldn't be the first
time...).
