I've only witnessed this issue on gnome 3.16.

If you bring up the menu by pressing the windows key, then lock the screen, the
applications menu is still visible on top of the lock screen, and you can still
use those items normally.

The only way to log in is to press escape so you can get rid of the
applications menu and bring the lock screen up, you can then log in normally.
