I've been trying to reproduce this with no success...

Installed 42.1 with this layout:

Snapshotted VM, then ran all updates.

Snapshotted again, started with a 42.2 DVD and ran an upgrade. After that, the
system came up as expected.

Does the block device layout match yours? How exactly was the upgrade performed
on your end?
