You're right, it's broken.

With keeppackages=1 all packages downloaded from that repo stay in the cache.
With keeppackages=0 (default) packages downloaded stay in the cache until they
are installed. Purging downloaded, but not installed packages after some time
does not work, so they accumulate over time.
