In my testing, so far, with recent versions of ESXi, the default SCSI
controller is the VMware Paravirtual SCSI controller.  There are some
differences in the way Linux enumerates the devices on a VMware LSI Logic
Parallel SCSI controller on bus 0.

So that will be my next round of testing - LSI Logic and either partitioned
disk(s) or LVM.

Are you seeing this on all SLES 12 SP4 VMs or just when SCSI0 is a LSI Logic
controller?

What is the VM hardware version of VMs experiencing this issue?  Is there any
pattern?
