Tested with Kernel 3.19.3.

According to google search results, the only solution is to compile the kernel
without USB_OTG. However, on this Ubuntu bug they say they fixed the bug in
3.18 - https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1411295 -- anyone
able to shed light on this issue?
