AFAIK LO checks for the presence of a dial option in the ppd-File of the printer. If that is presents LO displays a dialogue box asking for the target fax number when printing. I had a working solution with LO 4.3 from OpenSuSE 13.2 on my Linux PC (see https://bugs.documentfoundation.org/show_bug.cgi?id=89996). However, after some of the automatic updates the fax dialogue box failed to appear ... Unfortunately, I can't say precisely which update caused the trouble as I hadn't used the fax some time. You don't need a physical printer or fax device to test the issue. Just add a dial option to an arbitrary ppd-File and install a CUPS printer. Then the fax dialogue box should appear.
