Don't know if this is your problem, but I encountered a similar issue that proved to be a monitor that was going bad. After several weeks of symptoms, it died.

The thought crossed my mind but I tried a different graphics card and the problem never shows up. The other graphics card has another issue though so I cannot use it. The monitor is quite new too.
