No idea why it's trying to use ath when you have an intel chip. The
PRO/Wireless 2200BG's module should be:

modprobe ipw2200

do an lsmod and remove ath5k, mac80211 and cfg80211 first.

The ath5k drive in 11.0 is very flakey. If you actually do have an
atheros based chip, use madwifi instead.

Do an lspci to verify which chipset you have. Good luck
