Incoming mail is fine, fetchmail is also working ok.

Logs from your mail server would probably help. Along with postconf -n

It sees a connect from [::1]. Is IPV6 the enemy here??

Answering my own post:
yes, this was the culprit.
After adding [::1]/128 to mynetworks it is ok.
