Please don't load the fglrx module at all after boot. Even if it has been
unloaded it may have done some settings which don't agree with the radeonhd
driver.

Please test with a freshly booted system - without fglrx being loaded prior to
running radeonhd.
Please also remove 
 Option         "RightOf" "Panel"
to get clone mode for this test.
