Just to make sure: can you call conntest as described in the README?
Maybe we can fix both entries for good.

Outputs attached:
x1300-vga - rhd_conntest -s 1:0.0 (VGA cable attached.)
x1300-dvi - rhd_conntest -s 1:0.0 (DVI cable attached.)
7146.174B.0920.vga.rom - rhd_conntest -d 1:0.0

When I used the VGA cable, I wasn't getting a picture on the screen. I just swapped the cables while X was running, so I suppose that just means that the driver/card was already initialized to use the DVI output. If I understand the results correctly, it does appear that it was detecting the monitor, even though I wasn't getting a signal. Let me know if you need me to do anything differently.

It is possible (although I think unlikely) that this could also be due to the IOGEAR DVI KVM switch that I am using. The KVM switch occasionally

Not really. If you had DDC problems, yes, but hot plug detection uses
the same lines used for the RGB signal...

I suspected that was the case, but I wanted to mention it anyway, as this wouldn't be the first time I spent hours trying to nail down a hardware issue only to discover that the problem was caused by a KVM all along.
