Users are filling the form and it is outputed into a table. Each question into separate column and each submission into separate line. Nice summary of the data.
This is like SurveyMonkey? If not then what? Yes I can see 'results' are a by-product of the process, but GoogleForms seems to be an Android thing. If all you mean is a HTML based form that writes results to a text file or database on pressing 'submit", that's pretty old-hat HTML. BTDT and I'm sure many HTML designers have and I'm sure there are many OTS HTML libraries for it.
