[opensuse] Pulse audio: multiple sources, multiple devices

Is there some good guide on how to configure pulse to mix multiple sources
and use different devices (they are called sinks in pulse, I have USB
headset and an integrated soundcard)? I am in a desperate need of one.

Now it's a mess -- my applications fight each other over the control of
audio output. Kaffeine can loose it when put on pause and then crash when
run again, Gizmo (Skype-like communicator) won't even start, Amarok goes
mute. I need to kill Pulse and Kaffeine dozen times a day.

I would like the sources to be simply mixed (so that I could play music
while Gizmo is running fe), set default device for commonly used apps and
be able to change the device which is being used "on the fly". Is it
possible?
