I've had this same thing with 10.2 I think. I never found a cure, but
it was definitely connected with certain users. I used the GUI to
define who to show on the login screen. And there I found out that
there was 2 users when selected messed the list up. Including them,
made some others disappear also. But without those 2, everything was
fine.
I'm still running that system (with that flaw - fortunately, those
users are remote users) and just evaluating whether to upgrade to 11.0
or not.

FWIW, I just used Yast to create a new user and the icon is immediately available at the login panel. This is on 10.3
