You may not like this but it is possible that your HD is on the way out or is already gone.
No. I updated my 10.3 to 11.0 yesterday and it happened the very same thing to me. My hard disk was completely right: it was the before gfxboot app which was "disappointed" the system. I deleted and I got success at last. But this not before eight or nine hours working and trying to resolve the question. I must confess I was just about to give up SuSE.
