At the present time, at work, I do not use procmail. I just use a vanilla postfix with the built-in postfix lda. (And yes, postfix does deliver email to /var/mail/$USER (/var/mail is symlinked to /var/spool/mail).
The question is in the case where I am not using postfix, how does postfix set its locks. (The kmail default is /var/spool/mail/$USER.lock).

The locking method is compatible with the Postfix UNIX-style local delivery agent. My wife gets her email with kmail (delivered by procmail to /var/mail/Carol, /var/spool/mail/Carol) on my local machine/account and has never (I frequently check the logs) experienced loss or conflict. Kmail is set to get her mail from local periodically and I have set the lock to Carol.lock.
