usually a disconnect , or mis match of parts that used to work together.. It's been adapted from it's origional meaning by techies <grins> I *think* the origional meaning had to do w/ sound and what recording engineers used to call a "clam" noises that were hard on the ears, jarringly or grating on the nerves..
