Why would rescan-scsi-bus.sh see the DVD whilst the 6.4 boot sequence doesn't? Is there a problem in the driver?
I know I could put rescan in the local boot script but I would prefer to fix the underlying problem rather than cure the symptom.

I use the Adaptec 2940U2W controller and this seems to have happened after upgrading from 6.3 to 6.4, but that may be coincidental

Try sgcheck.
Also, it is possible that it is now being recognized as a different device name than it was with the previous version. If so, sgcheck will reveal it.
