Based on xface image that Sascha is using I made the one included in this 
post. 

I guess it would be nice to identify news guys at work :-) 

The whole procedure is more than simple. 
Click and hold the image in the email, drag it on a desktop, or any other 
folder, change name to something that one can remember, and then from 
KMail-Settings-Identities you can use Modify, tab Picture 
to add it to your KMail. 

It works that way in both KDE3 and 4. 

For those that need separate image news-geeko.png is attached.
