While you were doing your thing, I went out to my "kde_3.5_On_11.4RC2" SuseStudio creation (had to rebuild, thus the delay). Yes, when it comes up in its original form, the Kmenu "Settings" entry was not there. So I started poking all the .desktop files. This was too laborious, so I frustratedly right-clicked on the Panel > Configure Panel > Menus, and noted that "settings" *was* already ticked in the Optional Menus section, despite not showing up. Just doing nothing after viewing this, but saving, produced the Settings entry on Kmenu (and sub-categories, etc). So yes, Ilya, our approach was different, but same result.
