I haven't found a solution, my workaround is to try to remember the changes I desire, then apply them early in the session the next time I log in. For some reason the problem seems to occur more frequently the longer I've been logged in.
I have found that often the changes I have created will persist or become active at next login even though the "applying changes" window just keeps recycling, giving the impression that it has not finished.
I also see this behaviour when editing file associations.

I've got a system that hasn't had a clean install since 11.2, I've just acquired another hard drive so I'm going to do one and that usually fixes it. I once found a kde configuration utility through a configuration file in one of the kde dirs under /usr/data which fixed the problem, I'll have a quick look before I reinstall.
