The list of icons on the system seems to be fine:

I went trough all icons and I can't find anything similar.

The part of screen that shows gnome icon, even for add new widget window:

What I changed last was downloaded todays updates with
  zypper up -t package -r gwdg-KDE4_1
then deleted ~.kde4 directory in another user home that is used only to test 
KDE4, opened second session as another user, which created new ~.kde4.
