Weird.
Is the filesystem for sure ext4? Default is still Btrfs, so unless ext4 is explicitly selected I'd wonder why fsck.ext4 is being called at all. And if it is then you might need to make a new lost+found and make immutable, reboot, and then check the journal to see what's deleting it.

fsck.ext4 is being called by myself manually [ usually do it weekly ]
Seems excessive but harmless. Recreate the lost + found directory and chattr +i, reboot and then check the journal for "permission denied" or some such.
