My graphics card is a GeForce 8600M GS, and the nvidia website gave me NVIDIA-Linux-x86_64-185.18.36-pkg2.run for download.
Why are you using this driver version?
*argh* good question - looks like I should cleanup my downlaod directory. This file was "slightly" outdated. The fresh download from the nvidia website was newer, but...

For the records: I also got the "nv" driver to work with dualhead (starts up in clone mode at KDM login, then I was able to configure it to dualhead mode in KDE. The trick was to remove my xorg.conf.
